We have provided a high-level summary of some of these programs that may be available to your patients through their Cigna medical plan.*

Case Management
Special help and support may be available during the most trying times. Case managers work with you, your patients, and their families to assess each case and develop and coordinate a care plan that addresses multiple needs and engages multiple resources.
Specialty case managers focus on specific health concerns and utilize resources and activities that are custom-tailored to specific health concerns including catastrophic conditions, oncology, NICU, transplant, and high-risk maternity. There are many ways case managers may help your patients:
- Coordinate post-discharge care
- Identify community resources when needed
- Obtain health education materials and provide answers to health related questions
- Support them in understanding their illness or injury

Cigna 24-hour Health Information Line
The Cigna 24-hour Health Information LineSM offers your patients a toll-free number (1.800.Cigna24 or 1.800.244.6224) they can call at any time for immediate access to medical information and assistance. Features include access to nurses who may provide general health information about a variety of topics including preventive care, illness and condition overviews, diagnostics tools, and surgical procedures. Patients also have access to our audio tape library of more than 1,000 health-related topics to help them stay informed about health and wellness.
Services are also available for non-English speaking and hearing-impaired callers.
Well Informed in Gaps in Care
Well Informed is a clinically based program that analyzes your patients' claim data against evidence-based medical standards to proactively identify potential omissions or gaps in care. Examples include absence of lab tests and office visits, and medications they may have missed in the past. The program addresses disease prevention and focuses on more than 20 chronic illnesses and acute conditions including diabetes, chronic obstructive pulmonary disease (COPD), hypertension, depression, high cholesterol, and more.
The goals of Well Informed are to:
- Provide you with actionable information to help you manage your patients' care, increase their engagement in their own health, and improve your patients' health outcomes.
- Encourage your patients to collaborate with you and other health care professionals to take the appropriate steps to improve their health.
Healthy Babies
These benefits are available to most of your maternity patients who are covered by a Cigna administered plan.
Benefits include:
- A one-time educational mailing that provides prenatal care education and information about depression, stress, and dental care during pregnancy.
- Access to the Cigna 24-hour Health Information LineSM, where women can ask a nurse about everything from morning sickness to prenatal care or any general pregnancy questions.
Chronic Condition Management
With specialized assistance for those who need it most, this program focuses on common conditions. A dedicated resource takes a comprehensive approach by examining an individual's health, including behavioral, lifestyle, social and physical factors, to create a dynamic, integrated and custom-fit health engagement plan. Our chronic condition management program is supported by evidence-based medical guidelines and the most influential behavioral techniques.

Lifestyle Management Program
These programs can assist you by promoting early patient engagement, and providing support and coaching that can help your patients make lasting behavior changes to improve their health outcomes. Participants may choose between two options to complete the lifestyle management program. The first option is to complete several telephone sessions with a qualified health coach. The second option is an online program that offers a secure, convenient, and independent alternative for those who prefer a self-guided approach.

Health Advisor
This programs incorporates the total health support needs that have been identified and uses coaches to create one-on-one relationships to assist your patients in managing prediabetes, hypertension, and hyperlipidemia, or who want to focus on improving healthy eating and exercise. Health Advisors assess each individual's overall health status, health attitudes and behavior, and readiness to change, and helps with setting and achieving health related goals.
Cigna offers your patients pharmacy programs and services that focus on improving health, medication adherence, and making medications more affordable.
