I want to be aware of any restrictions I might encounter when prescribing for my patients. What drugs have restrictions?
On the prescription drug list, some drugs have symbols indicating that they may need prior authorization or have restrictions:
QL: Quantity Limits May Apply
Specific drugs have quantity limits to comply with manufacturer guidelines.
PM: Preventive Medication
Specific preventive medications may not be subject to the patient's plan deductible and/or copay (or coinsurance) based on benefit plan coverage.
PA: Prior Authorization
Prior authorization may be required. Certain prescription drugs require an authorization for coverage in accordance with an appropriate treatment regimen.
ST: Step Therapy
A component of prior authorization, step therapy may be required. Certain prescription drugs require the use of a pre-requisite medication before coverage for the Step Therapy medication is allowed.

If I order specialty medications from Cigna Home Delivery Pharmacy using the fax order forms, do I also need to request an authorization?
No. If you order specialty medications from Cigna Home Delivery Pharmacy using the fax order forms, Cigna Home Delivery Pharmacy will coordinate the authorization review for you.
Where can I find all pharmacy prior authorization forms?
You'll find pharmacy prior authorization forms (including authorization forms for specialty medications) on the Forms page.
What are the steps of the prior authorization process?
The prior authorization process begins with completing the prior authorization form and faxing it to the Pharmacy Service Center (PSC) at the number shown on the form. The review process may take up to 48 hours.
- If the request is approved, we'll fax you an approval letter within 24 hours from the time the decision is made. The PSC will put the authorization into our claim system to allow for coverage of the drug. Length of the authorization will depend on the diagnosis and information provided. We'll include this information in the approval letter.
- If the request is denied, we'll notify you through fax within 24 hours from the time the decision is made. A letter will be sent to you and your patient within 5 business days of receipt of the request, explaining the decision. The letter will include what your patient can do to appeal the denial.
